NOTE: FBN forms presented for filing must include all four copies (County Clerk-Recorder, Newspaper, For Bank or Other Agencies, and Registrant).  However, the County Clerk will file forms submitted without all four copies.  The registrant must then purchase any additional certified copies at a cost of $2.75 per copy that are required by Banks, Newspapers, and other agencies. 


 

Birth and Death Certificates may be ordered through our Fax/Internet ordering system or via Mail. 

The Birth and Death Certificate order form is an Adobe pdf Form that may be completed prior to printing the form.  All Birth and Death Certificate orders must be notarized before submitting the form for processing. **Notary Alert** New provision on Birth Certificate's and DeathCertificate's, Notary must include title within certificate of acknowledgement or the form will be rejected and returned.
